LSAC Account Number on PS/DS/Resume/etc?

I'm just wondering if I should put my lsac account number on every attachment that I submit? I've looked at three schools now and haven't seen those instructions explicitly stated, but have read on TLS and other places to include your LSAC account number. Should it be included on everything submitted, or only if a school specifically says to do so?
For everything submitted, should I put in the header what that document is? (i.e., Character and Fitness Addendum, Personal Statement, etc).

This also applies to labeling the document. In my header I have 4 things:
Name Label (PS/DS/etc) Fall 2016 Application
LSAC #

I included a header on everything except my resume as I felt that was a little redundant.
Here is the structure I abided by ---
Last Name, First Name Page Number
LSAC#
Personal Statement/DS/Addendum/Optional Statement/Why X

Unfortunately I completely forgot to put the LSAC Account number on the Personal Statements... boneheaded - and now already submitted. Any idea how this will affect my application or what my play should be?

Nothing to do now. If the instructions didn't say to add it you're fine...if not, you just look like someone who can't follow directions. Luckily they don't have to report that to the ABA or USNWR.
