For the particular school(s) you're referring to, the safest bet is to just ask their admissions office.
I don't recall including any supporting documentation for C&F disclosures during the law school application process. That being said, for the C&F process to get admitted I had to produce copies of the documentation, so it might be worthwhile to spend the time attempting to track down any records that are out there now to save yourself some time in the future.
