I have noticed that in many applications there is a section asking for an explanation of any interruptions (usually of periods over one year) in your education. I think if you explain it there, an addendum would be unnecessary. (Don't say you were too lazy lol, just say you were working and your situation didn't allow you to complete the requirements until this year.)
