Typically, an academic letter of reference should be from a professor at the university level who has had you as a student. If it was a Spanish professor at a university who has had you as a student, it may be sufficient, but if it was a personal tutor, that may be something to reconsider or ask admission's departments of the schools you are applying to. Not to say you are, but don't be embarrassed about asking professors for letters of recommendation, they get asked this question all the time.

If you have never had the professor in a classroom setting and only interned or done clinical research for that person, that seems more like an employer-employee status than academic reference from a professor. Remember, the person you ask to write the letter, will likely describe how they know you.
