Can anyone link me to a good C&F Addendum template? Or lead me in the right direction as to how to go about explaining past run ins with the law? Other than saying I pleaded guilty to X on Y date and I learned from it, I'm a little lost on the necessary things to include and the overall structure of the addendum.

Ok so in my case there are two different incidents I need to disclose. Would it be better to go about it by stating "I pleaded guilty to a on x date and b on y date" and then go about explaining how remorseful I am and how I've grown, or would you recommend I go about addressing each one individually?

For reference, both incidents were over 5 years ago, making me more inclined to group them together and state that I am more mature now, etc.
