I received quite a few fee waivers in the fall of 2012 - spring of 2013 before deciding to apply this cycle instead. Are those fee waivers still valid, or should I have received new fee waivers for this application cycle?

I'm fairly certain fee waivers are cycle specific. When you get to the payment part of the LSAC app, the price should tell you if the fee was waived. If it wasn't, email the schools in question, mention they gave you a waiver last year, and ask if they'll give you a new waiver.
