C&F Addendum Critique

I realize this is about the billionth one of these posted but if anyone could give it a once over I would really appreciate it. I am a little worried that the combination of incidents shows a bit of a pattern. Thought about expanding on this a bit but the general consensus around here seems to be to keep it as short and concise as possible. Also, am I correct including all C&F issues in one addendum or should I submit three [addendum ] separately?
On August 23rd 2008 I was charged with reckless driving (failure to drive right). Following the completion of a court order probationary period the charges were dismissed.
On Date X, 2009 I was issued a citation for Minor in Possession of alcohol at the University of XXXX . I completed the course required by the university and the charge was expunged from my permanent record.
On June 9th, 2012 I was arrested and charged with driving under the influence of (DUI) alcohol and hit and run property damage in Hometown, State. Following the arrest I plead guilty to reckless driving and hit and run property damage. I completed all court order treatment and evaluation requirements and it was determined that this incident was not indicative of a larger substance abuse problem. This incident was a spectacular lack in judgment for which I am extremely remorseful. The entire process has been a very humbling learning experience; I look forward to putting this incident behind me moving forward with the next chapter of my life.
