A GPA addendum is understandable because you can no longer go back in time and redo undergrad... I think it will be hard to write a convincing LSAT addendum that doesn't simply sound like a lame excuse because you can retake the LSAT at any one of 4 given times throughout the year.

Unless you have a legitimate information-processing disorder, I cannot think of a single good reason of why you should write an LSAT addendum. (Even if someone came into the testing room and started running around shouting things, although you can complain about it, the right answer is retake and prove that you're capable of a higher score.) If you do not have a disability but still want to go through with the addendum, please ask this guy for help.

EDIT: Unless, of course, it is an addendum for a more-than-six-point increase in your score. If it is just an addendum explaining why your score is low, it is probably a bad idea.
