Top Law Schools

I feel a bit confused with the recommender and evaluator shown on the LSAC CAS webpage. From what I know, the law schools that I'm interested in only require LoR except that UCLA says they require two LoR and accept one evaluation form. Because I want to minimize the confusion that my recommenders may have, I would like to find out if you guys request both LoR and evaluation from your recommenders?

Only do the LORs.

The evaluator thing doesn't seem to be taking off the way LSAC would like. The schools don't want it. Don't put your recommenders through the bother of doing it.
