Is it necessary when you ask for a LOR that you provide the professor with a cover letter?

It's not necessary, but you should. Write a few 'memories' from class that will trigger them to write about a certain quality you have (if you want them to mention class participation, mention the in-class debates; if you want them to mention your writing abilities, talk about that paper you enjoyed writing).
