Hunting down my last LOR

Alright, so I'm trying to track down my professor who said she was going to have my letter done by now. I'm thinking of sending this e-mail... tell me if it sounds too passive-aggressive.

Hey, not trying to bother you since I know you probably have a million midterms to grade as well as all of the other hats you wear, but I was just wondering how the letter is coming. My score is coming on Friday and I was hoping to get my apps in right away, but if it takes awhile I understand. If you need anything else from me, or if you need me to resend the form from LSAC, just get back to me via e-mail or call me at 867-5309. Thanks again for your time. I know you're busy and it's very valuable.

I have a close relationship with this professor. She was my mentor for a research fellowship that lasted over a year, and I took three of her classes, all of which were small and discussion based. I've met her husband and kid, so I'm pretty sure she's not trying to get one over on me. I don't want to come off as a dick, so yeah... is that firm but fair?
