Sending in your LOR yourself

So I was able to get one of my professors to write a LOR of for me. Unfortunately, I didn't read the LSAC guidelines for submitting LORs first. Stupid, I know. I have the letter in hand, but didn't have her fill out the LSAC form that you have to send in with them. Right now I'm just thinking I'll print out the form, drop by her office and ask her to fill it out and then pair it with the letter and go mail it myself (I want to inconvenience her as little as possible, thus I'm not going to ask her to go back and submit it electronically). Does it matter if the return address on the letter is my own personal address? I know the LSAC website says the recommender should mail it, but if the letter is signed by her and she's filled out the form, do they care who physically mails it?
