Do any of the T-14 schools require an evaluation or just LORs?

I'm not sure, but since no one else answered, I think your best bet is getting the professors who write you LORs to just do evaluations too. That's what I did and for the schools that allowed me to send more LORs than I had, I sent them evaluations in addition to my LORs.
