What header should I put on my personal statement and diversity statement? Name, LSAC Number, and what else....?

In the header:
Name
LSAC number
"Personal Statement for X Law School" or "Character & Fitness Addendum for X Law School" etc.

In the footer:
Page number ("1 of 2") for anything with more than 1 page.

Put page # of total. If a page falls out of your file and they start reading your essay, you want them to KNOW there's a page missing, not just wonder why you broke off mid-sentence.
