should I put this for awards section?

For the list academic awards, honors, recognition section would I put the following:

winning chess tournament (intercollegiate)
winning debate/ranked as top speaker
winning math competition (it was a competition held by this math club from college)
should I put down the two or three times I was on the deans list with dates? What would I put for the explanation..

What else goes in this section? Can someone tell me what they put, just so I can get an idea of what kind of stuff goes in here?

Also how would I write this, bulleted list? Essay form?

And, how do I get a fee waver?

1. Intercollegiate chess tournament champion, Ranked as top speaker in xxxx debate, . . . . and so on

Don't stress it too much, because they will probably skip over this and look at your resume.

2. Email the school and ask for a fee waiver.
