Dates and Disclosure addendum

How important is it to have the exact dates for a disclosure addendum?

I have two underage drinking citations I received during my frist year of college. I have no idea what the dates are nor do I have a copy of the citation. Do I need to find away to attain those dates or can I just say something along the lines of...."During my freshmen year I received two underage drinking citations...."?

You might as well make a good-faith effort to find the info.
Were the citations from the police or the school?
Contact the police/courthouse or the school to see about getting a copy of your record.

If I received a drinking ticket from my school I have to report that? Such as a ticket for drinking in my dorm room?

Schools require disclosure on different things. Some want to know about any disciplinary actions by the schools, some don't. Read the application instructions carefully. If in doubt, disclose. Disclosing will pretty much never hurt you.
