Resume question

Different schools have different guidelines on the resume. For example, one school wants to have dates and devoted time for each item.
1. What do you do if you don't remember all the dates? (e.g. work or awards that were done/received awhile ago)
2. What do you do if there was no set devoted time and it fluctuated widely? (e.g. intermittent volunteer work)
3. Do you think leaving out this information for uncertain items is acceptable?
Not sure if putting in a rough estimate is better than not putting any detailed information at all.
