How to write a disposition

I googled this and couldn't find anything meaningful
lets say this is my record

arrested for "A" : dismissed
Arrested for "b" : convicted of "c" sentenced to conditional discharge
Arrested for "d" : convicted of "e" sentenced to pay $100 fine

Is this the way to list my dispositions?
Should i mention that i received youthful adjudication? I believe that is similar to a sealed record.

XYz 98978 (XX/XX/XX convicted for Y)
ABC 98658 (conditional discharge after completing 789)
• GHJ 98758 (X/XX/XX convicted and sentenced to 456) Adjudicated Youthful Offender
• ABC 91243 (X/XX/XX dismissed)
• DEF 96837 (XX/XX/XX conditional discharge upon 123) Adjudicated Youthful Offender


Is this format adequate?
