The problem is that it says that I "Must" include such a document and that my application is not considered complete until this is done so.

Why don't you contact the admissions office of the schools you're applying to and ask them? I'm sure they will be able to tell you what to do. Many schools destroy all disciplinary files of students upon graduate, save for academic probation and dishonesty, ect.

My next question is this: The last school I called said that I should include in the addendum a scanned copy of the exact regulation from the code pertaining to how records are retained. Would it hurt me if I did this for all schools?

No, and you're better off doing it for all schools to cover your bases just in case.

Are they requiring "a document related to the final disposition" or "a document from the school related to the final disposition"? If they're asking for the first kind, your statement should be sufficient. If they're asking for the second kind, I would start with contacting the dean’s office at your UG and ask someone there to write a letter stating what you said or stating that you have no disciplinary records on file.

Not sure if it is the same set of schools as in my situation, but NYU and Columbia both ask that your University send a detailed explanation of any disciplinary action. I had a similar thing where my school said they did not have any records to send, and I emailed the the admissions offices about it. NYU never responded, which was kind of weird, but Columbia just said to have the school send a letter saying they had nothing to report.
