Where to put LSAT addendum for Georgetown? Forum

Where to put LSAT addendum for Georgetown?

I was a little sick the first time I took the LSAT and had a 6 point increase when I retook. Should I put the addendum under Georgetown's Optional Response category, even though it seems that they only want answers to one of the 5 responses? Or should I attach it to something else like my personal statement?
