What should I include in my addendum?

Hi guys, I took the June 2011 LSAT and due to a medical issue, failed to live up to my PTs. It was nothing major health-wise, but it was enough to screw up my score. I retook in October (without the issue) and nailed it. That said, I am not very worried about this aspect of the addendum.
I'm mostly concerned about the score I canceled in December of 2010. Prior to the test, my employer began increasing my responsibilities and hours at work. I decided to stop studying, cancel my score, and take a year off to consider my career options. I ultimately decided to pursue law, studied my ass off, and (eventually) got the score I wanted.
Here are my numbers:
LSAT: Cancel, 167, 177
GPA: 3.76
Should I address both the jump and the cancel or just the jump. My friend (a T20 law student) said a short addendum on the jump alone would be more effective and avoid the appearance of "making excuses." Made sense, but I wanted to throw it out to the forum. Thanks!
