Was this your only cancellation? If so, don't even bother with an addendum. adcoms know shit happens (hahaha really sorry i didn't even mean that as a pun but now i have to leave it in) and so one cancel doesn't raise any eyebrows. in fact, though i might be remembering incorrectly, i think in her book anna ivey mentions food poisoning as a potential cancellation excuse which you should not discuss in an addendum.
