i had the same problem. i called LSAC about this and apparently it is a common problem that they are trying to fix. the guy i talked to said there was some issue transferring the info from the app into a PDF. he said they are working on it and to wait transmitting more apps until the preview shows up like it should.

Same here--although it's weird that only one of my apps this did this. I just printed out 150 pages from all my other apps to neurotically check if anything was left out there...
