If you have two scores that are more than five points different.

How often is hard to say, but LSAT addenda are usually written to explain a low score, whether it's due to unusual circumstance (such as test center issues, illness, acts of god, etc.) or just poor performance the first time around.

I'm 12 points different and not writing one.

This is just what the admissions people from Stanford and Harvard told me. But, to each their own I suppose.

I had a 15+ point difference and I sent one to all the schools I applied to.
Hopefully they won't ding me for sending something they didnt specifically ask for.

If you are unsure, just contact admissions.

I was very careful throughout undergrad to keep my GPA low enough to be unable to get into either of those schools.

In all seriousness, my only "excuse" was that I didn't understand the critical nature of the test and the ridiculously competitive legal job market the first time around.

They're written more often than they should be.

As someone else stated, some schools will specifically ask for one if your score increases by a certain amount. In that case, you should write something; however, it's probably not going to be very compelling, and the schools probably won't care too much as long as it's just two scores.

The only effective LSAT addendum I have ever read was written back when schools tended to average two scores. The student in question had a SIGNIFICANT jump (+15 maybe? I might just be making up a new number each time I tell this story, but it was a big jump). That student was a recent immigrant and new to the language. In between the first and second LSATs, she took a very intense English language course that significantly improved her English abilities. The essay was also beautifully written, reinforcing the, "I now speak English very well," theme.
