I am really confused if I am suppose to be submitting multiple addendum's or if I am suppose to combine them. The explanation for all of them is the same but on applications the questions come up separately and ask for different attachments.
Maybe it'll make more sense if I explain the situation. My GPA isn't representative because I failed at my first attempt at school. I received two Ws and two Fs and because of that was also put on academic probation and then decided to take time off of school because of this. They ask for a CF for the probation on one part, the reason my school was interrupted for one or more terms, and then I also want to explain what happened that term with a GPA addendum. It just seems as though I will get quite repetitive. Also it's a bit frustrating that the apps ask these things in so many ways.

I'm no expert but I wouldn't write a separate addendum for a single occurrence.

I would just write an addendum entitled: Addendum of Leave of Absence, Academic Probation, and Whatever Whatever"

Then write about the circumstances.

Yea, one addendum is my vote too. If separated, the two will have almost the same story, but a mildly different conclusion that is not worth separating. Oh, and by the way,the plural form of addendum is "addenda."

I had to write all three of those too, I put the question number for each different school and then the explanations. Mine kind of all built off of each other.

Also, even if the plural of addendum wasn't addenda, it wouldn't be addendum's because that means "belonging to the addendum" as and apostrophe followed by an S denotes a possessive. I know it's petty grammar Nazism, but it's a pet peeve and your punctuation should be flawless on all of your admissions materials (well, on everything really, but especially admissions materials).

Actually, those two are exceptions to the apostrophe convention and are contractions, not possessives. Unless you were being sarcastic.
