When I was in high school, I took a class that was a college in high school class and received credits for it through seton hill university. I then transferred the credits for the one class to the one I attend now, Duquesne University. Do I have to list seton hill for my transcript or is it fine since the credits show up as taken from Duquesne?

Seconded. Just FYI, it will count as equal weight in your LSAC GPA.

Do you mean it will just count as a normal three credit class?

If it counted for three credits of transfer credit, it carries the same weight as any other three credit class WRT gpa.

After you add them you need to print out the request form and give it to the school. The school will then send your transcripts and the request form to LSAC.

Ok, I went to Seton Hill and handed in the form and they are processing it. The only issue is that in high school I would sometimes spell my name with an apostrophe, O'Masta. I then realized that the spelling on my social security card spelled it without an apostrophe and I stopped using the apostrophe. Unfortunately, seton hill has me spelled with an apostrophe and the lsac and their paperwork has no apostrophe. Should I call then and inform them of this to make sure they accept the transcript?

It couldn't hurt.
On another note, I do know that the name on your LSAT ticket and the name on your driver's license needs to match when you go to take the test.

The form you send to Seton Hall has a barcode on it, identifying it as yours. The barcode will match your LSAC account and the name will be close enough. I don't think you need to do anything extra like call.
