Is it an issue you're 100% sure you'll need a form for? Either way, you could send the schools an email asking if it is and where you can find the form, because not all schools have them (and just want you to send a report from your ugrad).

I'm in the same boat. However, in the T14 apps I looked at, the only school requiring a dean's certification was Cornell. All schools required an addendum.

Thanks. I saw in NYU's last years application, they require a statement from the university so I assumed this was a dean's certification.

I think I'll just email ALL the admissions offices and confirm to make sure. It will be a pain but I guess worth it.
Ah, I didn't look at NYU's since I'm not applying there. I checked Chicago, Michigan, Penn, Virginia, Northwestern, Cornell, Georgetown, Illinois, WUSTL, Notre Dame, George Washington, Minnesota.

No, NYU does not require a dean's certification.....

Yale had a dean's certification last year, but it looks like they're dropping the requirement and requiring it for only admitted students.

Stanford still requires it.

And for YS, they were separate forms, yes.

You can just email the NYU office of admissions and ask.

You can also find these forms sometimes on the website and download them (assuming the school accepts non-LSAC processed applications).

A number of schools will ask for them after admission, not as part of the initial package.

e-mail the schools you're applying to. i just had someone in the dean's office write a note about my disciplinary record (which my UG actually expunged upon graduation, so there was nothing for them to actually report).

Y and S have the dean statement forms that you need to fill out regardless of whether you had a disciplinary issue or not (though Y moved it to after admission i guess). most schools don't have a specific form in the T14, although I didn't apply to all of them so I can't speak on a few.
