The posts saying Cornell requires one are from 2008/2009. For this cycle, at least, it's not required unless you've had some kind of disciplinary action or something similar. From the application:

Have you ever been the subject of disciplinary proceedings or been warned, placed on probation, dropped, expelled; or been asked, or advised, or permitted to withdraw; or been suspended for academic, nonacademic, or any other reasons by any of the colleges, universities, graduate schools, or professional schools you have attended, or are any such proceedings pending? (if yes, you are required to submit a Dean's Certification Form from the institution where the infraction occurred or is pending)

Cornell only needs dean's certificate if you answered yes to disciplinary questions...you should point this out.

University of Washington scrapped their dean's certification requirement this year.

This can't be true since it says the EXACT opposite on the application. Someone ban this troll.

USC requires a dean's cert if you answer yes to a disciplinary question. I have my application marked as incomplete pending receipt of one from my school (annoying because the records have been expunged).

Suffolk doesn't require one anymore. Unless I am missing something badly.

I think these are obnoxious.

Does anyone know where to get a USC Dean's Certification form? They don't have one attached in their application and I couldn't find one on their site either. My school won't fill out a Dean's Certification for me unless I fill out a request form from USC first...

Hey, OP, I think it may be helpful to clarify which schools have actual forms and which require letters from the dean.

NYU, Columbia, and Duke all require a letter from your dean's office if you answered yes to undergrad C&F but do not have a form.

Also does Stanford have their own form? I'm going to request my school to send ones to NYU, Columbia and Duke because they don't have specific forms but I'm not sure about Stanford, Cornell, Harvard, Yale etc.

i'll specify that this is because I have to answer yes for disciplinary action, not required of everyone (except stanford).

Looks like Duke, CLS, NYU require them if you've answered yes to C&F. What about Chicago, Penn, UVA and Harvard?
