I was filling out the primary application form for my Columbia app, and realized that they want you to list your
extracurricular activities, and other softs on 4 lines with no columns or headings.

Would it be sufficient to just put something down in the manner of: a member of XX club, looked after mother
who was in poor health, worked as a sales clerk at a local drug store. ?

do I need to put the time period, or provide names of the employers?

Are you also attaching a resume? If so, you can say "please refer to my resume for additional details" such as time period, etc.
