How do I format Essays?

Do I need to follow any sort of strict formatting for name, LSAC #, type of essay (either in the header or footer)?

I put my LSAC account number in the header of my essays. That's it though. No need to put your name.
Theres no special formatting. Just write an essay.

FWIW, In my header I had Name and LSAC # (left oriented) and Title of Essay (e.g. "Personal Statement" or "Why X?") and Page Number (1 out of 2, 1 out of 3) (right oriented)
I think page numbers (especially doing "page 1 out of 3" etc.) is important just in case a page gets lost or misplaced, they don't think your 2nd page is the last page if the 3rd goes missing.
I realize that not all adcomms waste the ink/paper printing out admissions packets but on the off-chance they do...

The most common header I've seen is:
Eskimo (left aligned)_____________________________________Personal Statement* (centered)______________________________________LXXXXXXXX (right aligned)
*Just "Personal Statement" or "Why X" or "Addendum"
