To me it sounds like you are trying to dismiss them completely and not take responsibility- while in your shoes, that's probably exactly how I'd feel, I'd just be careful about how it will sound (to a potentially older, more boring teetotaler)

This addendum seems like a series of excuses. Even with Type 1 Diabetes, you're a traditional student, and you would be under the legal age of alcohol consumption. All you state is, "Oh by the way, I was on probation because I was underage."

Your statement makes it seem like your incidents are merely accidents. I think this will hurt you in the eyes of the ad coms.

Can I be more general and say.. "During my freshman year of college I was placed on disciplinary probation after two incidents of underage drinking. There have been no further incidents since freshman year. "
Well I like the first one, I would just maybe add something like "While I take responsibility for this unfortunate incident, I learned from this incident that health conditions require me to be more careful in my consumption of alcohol." And for the second I'm not really sure how to explain that. Having been on the other end of that situation my reaction is "Sooo.. you're telling me you got so drunk that when you passed out your friends were worried you weren't going to wake up? How drunk WERE you?" So yeah I might not mention that part, and just say, "After one more incident involving alcohol, I truly learned my own limits and the importance of responsibility, and there have been no further incidents."

Kinda corny but I think it's important to stress that you take responsibility for it and that you learned from it.

Eta: I wasn't thinking about the fact that you were a minor.. hmm.

Yeah, I can see that now.. what do you think about my more general post? Do I need to explain it in more detail than that?
In my addendum, I stated the basic facts of the incident and then followed with a short reflection on how I grew or gained from the experience. Be sure to keep it brief and succinct.

I wouldn't mention the diabetes.

I think the short one you posted up earlier should be fine. Just tack on a "Ask me for more details if you want" thing at the end.
