Addendum vs Addenda

Can I say title the document addendum even though I have two issues I bring up? Or do I title this Addenda?

If all remarks are contained in one addendum, then it should be referred to as an addendum regardless of the number of issues addressed in that one supplement.

i would title it for what the addendum is about. For example, if you have an addendum for your poor lsat, write addendum for lsat as title. If you have one for college disciplinary issue, title it addendum for college incident or something. If the events are related ( disciplinary issues, gpa, illness/ why you took 2 semesters off, then you could put them on the same page. If they are unrelated ( one disciplinary issue, and a lsat score gap) write 2 seperate ones.
