If you have a good reason then send one everywhere.
I only shot up 6 points and I still sent one out, because I was ill during the first one.

The only place to take the LSAT in the country where I live is 430 miles away (the equivalent of DC-Boston)...was able to take an extra day off of work (wasn't an option the first time) and take the bullet train instead of the night bus, getting me there farther ahead of time.

From Penn:
RP: Write the addenda! You don’t want the Committee guessing about what may or may not have happened. We have wonderful imaginations. Be brief and articulate when writing your explanation. Use your judgment when determining how many explanations to write.
So I'd also do one there...
Also I think for an addendum to make sure it gets included I'd write a word document with a heading with your name, ID # and just title it LSAT addendum - then attach it as a supplemental file after resume and personal statement

I had a 10 point jump, a pretty legit reason, and submitted with all my apps as an addendum. Submit as addendum to your app. If you've already submitted your apps, then email them the addendum and they will add it to your file.

I researched just the schools I'm applying to (including a couple of T-14s) and they all wanted an addendum for a "significant score boost." My reason wasn't as good as yours, but I still made a decent sounding one since they asked for it. I went 157-172, so yeah...you should addend.
