I respectfully ask that the admissions committee focus on my most recent score of 167 as a better indication of my performance than my previous two attempts. Since originally taking the LSAT in June of 2009, I was diagnosed with [disease]. I had struggled since late 2008 to find a diagnosis for an array of symptoms and saw a wide range of specialists before I was finally treated in the winter of 2009. In retrospect, I should have waited longer after treatment before taking the test a second time in February of 2010.
My doctor has confirmed that I am healthy, and I am excited and ready to start law school.

Don't really think the contact info is all that necessary. Just let them know you had an illness which contributed to you not doing as well as you should have the first 2 times and that the 3rd time is most indicative of your true abilities.

It is not appropriate to include the doctor's name & phone number without a signed consent to disclose form.
