The grade trend itself doesn't warrant an addendum unless you have a valid reason, but it looks like you do.

This sounds like the ideal situation for an addendum. I would even word it as you really wanted to serve your country and pushed yourself to finish the year despite the medical problems but in the end could not complete the program.
