Freshman year GPA ~3.48, post-freshman year GPA: ~3.94. No specific reason other than the typical transitional period. However I do think I have changed significantly since them. Is this really worth including an addendum for, or do I just reference it in PS? Hope they notice the upward trend?

No. A lot of people have bad freshman years. It's not worth calling attention to it unless you have a valid excuse.
