Can someone read my addendum for being on probation

I think it's good, short and to the point. You may want to include something about "Since this incident I have not been subject to any more disciplinary action by my undergraduate institution." or something like that. You could also include a sentence briefly describing what happened.
I think you've got the right idea though.
