Top Law Schools

One of my recommenders is asking me how the letter should be set up in terms of who it should be addressed it, what information to include about herself and whether its necessary to include the lsac mailing address on the actual letter. My instinct is to say:
1. Include all her contact info
2. Address is to something general like: law school admissions committee
3. Do not include the lsac mailing address

Is this right? Can anyone post a general format of what they have seen? Did I forget anything? This is a general LOR that I will be sending in all my apps.
