Dean's Certification question

I'm applying to Suffolk and I find their Dean's certification instructions to be very confusing. I'm not sure if all dean certifications are the same or not, but if they are maybe someone can help me. At the bottom of the certification there is a place for a name and address and I'm not sure if I'm supposed to fill it out or the Dean, does anyone know? Also, do I mail it to the dean in an addressed envelope to him on it or do I hand it to the dean with an addressed envelope to suffolk attached? Thanks!

just sent my suffolk one out this am!

i filled out the top, then sent it to my dean. they are to fill out the bottom and send it to suffolk. its like the rec forms, just waiving your right to see what they put.
