how do you find out what's on your UG record?

Is there a way to get a full UG report which would include any disciplinary actions that may have been taken? I wanted to know where do I go to get this full report? I wanna see what exactly is in my record.

You probably need to contact the registrar's office in your UG institution to find out.

If you want to personally see your disciplinary record, it's probably best to sit down with someone in your student judicial affairs office (or whatever it may be called at your institution). I did that to review my info prior to submitting apps, just to make sure I had all of my info straight and didn't mistakenly leave something out and seem like I was hiding something.
That kind of stuff is not typically on any transcripts. Schools are not in the habit of releasing that kind of info without your permission, that's why most law school forms you submit for a Dean's Certification have a waiver signature so your UG institution can provide it.
