My GPA is 3.515, but has an upward trend except for 1 D from a 1 credit internship I did where my supervisor did not like the fact I could not work on Sundays, and complained about handing out fliers and sweeping. Should I attach an addendum so the adcomms don't think I'm a slacker or will this seem bitter?

If it is a 1 credit D surrounded by pretty much straight A's, I'm pretty sure they won't judge you on it. I would say no.
