Letters of Rec - Supplemental forms

For letters of rec, did anybody look at the individual schools' supplemental recommendation forms to see what qualities your recommenders should be addressing? Should I wait until applications and these forms come out for Fall 2011 to have my recommenders start on the letters or should I have them start ASAP using letters of rec guidelines in law school admission books?

I think a couple schools even have a checkbox grid to rate you on these individual supplemental recommendation forms? Did you guys have your recommenders fill out a grid like this? Did you send the individual supplemental form with the grid to your recommenders?

I am kind of interested in this, too.

I am having my LoRs sent to LSAC right now (Specific letters to Cal and Stanford, then a general letter to all the other schools I'm apping to) but I am also concerned that I'm MISSING something!

Will we simply not know about supplemental items until applications open up for Fall 2011 matric?
