Where in your LSAC account do you do that? Word or PDF format, or something else?

I know I'm being lazy, but I figure someone here will have a quick answer.

You can attach it individually to each application. I did this, and modified each addendum slightly for each school. Or you can save it in your profile and attache the same one to each. Just add it in the same way you do your PS.

Word is fine.

When you're attaching documents you just upload it and attach it where you upload and attach your personal statement.
