Expert opinion requested: it is only 38 words!

I'm not sure how to approach this, and I would appreciate some help, stylistically and or grammatically. Quick background, "step-dad A" is the topic of my PS. He was/is a rough character, a drunk, and a complete fuck-up. "Step-dad B" went to the same law school that I'm applying, and he is an active alumnus. I’m 100% positive the adcoms will know him the second they read his name. This has got me so concerned that I’ve considered not applying. Step-dad B is well known within the legal community, and I'm highly concerned there might be some confusion, even the slightest would be an issue. So I decided to add this quick addendum:
It is imperative that the distinction between my step-fathers is clear. The step-father I refer to in my personal statement is purposely left anonymous, and [name] an attorney practicing in [city] is not confused with the anonymous step-father. [name] entered my life in my late teens, and married my mother when I was an adult. [name] is a wonderful role-model and mentor. [Therefore, I believe this addendum is necessary.?!?] <--- yea, I know cut it.
Additional questions:
1. Also, I’m writing a few short addendums, should they be on separate sheets.
2. And, if so, should a salutation be included on each?
3. Should a salutation be included at all? The instruction state: clearly list the question you are writing about with name and LSAC number.
