2 Addendums....killer?

Lets say you have a medical reason for a problem with both a certain aspect of your GPA and one LSAT score (out of two). Is it worth it to write an addendum for both or should you pick the one that you believe will have the greater impact on your admissions and write on that?
For medical reasons do you attach a letter from your physician or just the brief explanation?

Write one about the medical issue and mention both effect on GPA and LSAT in it. No need for documentation unless requested by the schools.

Couldn't hurt to include a sentence that says you would quickly provide documentation if they asked.
