How did you format the heading for your essays? Right now, I have it formatted like below

Name
LSAC #: XXXXXXX
Personal Statement

But how have you guys formatted your Why X essays, especially for schools who did not have a specific prompt for it? Could I label it as "Supplemental Essay: Why UVA" or is that too casual?

Your heading sounds fine. Just label it however they have it labeled and make sure it looks professional. If they have it listed as an optional statement, I would write "Optional Statement." They will know instantly what it is when they start reading.
