Please Critique C&F Addendum

Please don't quote. This is my draft of an explanation of a C&D. I appreciate any advice.
I am writing this addendum to address an incident that happened early in my second year of my degree at the University of College. This occurred approximately in October 2009, as there were no formal charges no documentation currently exists for an exact date.

In the one of my classes that I took we had access to the solutions manual for the homework assignments. On several occasions I accessed this manual to assist me on the assignments. After several weeks this became a widespread issue throughout the class and the professor confronted the class saying the solutions manual represented an ethical violation. Students whose homework had been identified as using the solutions manual were spoken to by the professor and received a reprimand. As I had not copied from the solutions manual I was not caught and did not initially receive a reprimand. I later spoke to the professor about my usage of the solutions manual and submitted to the same punishment as the students that had been caught. After admitting my guilt I received a signed and sealed warning in my file for the duration of my degree program. This letter stated that any further ethical violation would result in an expulsion from the program. After graduation this letter was removed and not reported to the dean.

The message/content looks good to me. The grammar, punctuation, style, etc. in the writing could use some polish.

Thank you! I definitely have some rewriting to do to make it more presentable. I hadn't realized that I would need a C&F statement for this incident, so I was a little caught off guard on having to write something up.
