Stupid question

How exactly should your PS be formatted when you submit it? I know it's uploaded as a word document but do I need a header? Page numbers? Title? Should I put my name at the top with my account number and date as if I'm turning in a paper for school? Just want to make sure I get the logistics right.

It's been a very long time, but I think I just had a header with "Personal Statement for [name]." You could probably throw in your LSAC number too. It's pretty much up to you.

I put headers on all my attached written documents (other than my resume), as well as page numbers on my PS. No dates.

e.g.:
Full Name - LSAC ID # xxxxxxxx - "Personal Statement" or "GPA Addendum" or "Diversity Essay" etc.

And I put the headers in italics. Personal preference, but I think it looked clean.
