In my opinion, no. I think an addendum for your situation is unnecessary. From my understanding, scholarships are offered based on your hard UGPA and LSAT scores, without consideration to circumstances for why either are lower than "you" think they should be (I would assume this would only help with borderline admission scores).
However, I don't think an addendum would hurt your situation if you added one, so there isn't really any downside to doing so if you truly feel you need to explain one slump semester.

No. This will just annoy them. USNWR doesn't factor addenda into the rankings.

You should be fine without an addendum.
You may get a bit of a break for having a STEM degree.
You should also check out your Credential Summary Report (I think that's what they're called) on LSAC. Checkout what the median GPA for your undergrad is and where you stand.
Different schools have REALLY different GPA curves. You could be somewhere in the top 20% of applicants from your school - and that would include the dance and basket weaving majors.
