About to submit my applications, just wanted to make sure this GPA addendum is ok. My GPA went from a 3.7 to a 3.56 as a result of this semester. 1 D+ , 1 C-, 3 B's, 2.5 GPA for the semester. The addendum is below.

GPA Addendum

In September 2011, I was involved in an automobile accident. As a result of the accident, I suffered a concussion. The symptoms of the concussion, mainly headaches, were severe at times. This led to me missing a lot of school during the fall 2011 semester. It is my opinion that this led to my poor performance during this semester. A copy of the crash report and medical documents can be provided upon request.

IMO, looks good.

This may be the only solid GPA addendum I've ever seen in this board.

Only change I would make is to "missing a lot of school." I'd do something a little less conversational, like "missing many/several classes."
